Children and adolescents are highly exposed to harmful polycyclic aromatic hydrocarbons (PAHs) that exist as mixtures of hundreds of compounds in air, soil, water, dust and food. This study investigated 21 personal, lifestyle and environmental predictors of urinary PAH metabolites from four PAHs (naphthalene, fluorene, phenanthrene and pyrene), using a dataset of 516 3–17-year-olds from 167 locations in Germany. Spearman correlation analysis showed weak to moderate associations (ρ = 0.2–0.6) between PAH metabolite concentrations. Regression analysis revealed high nicotine exposure, young age, and residence in large cities to be the most important predictors of multiple PAH burden, with estimated changes in metabolite concentrations of 124–1080%, 47–155%, and 22–57% respectively. Other notable predictors included: BMI, socioeconomic status, sex, consumption of chocolate, smoked and barbecued foods, chewing on plastic objects, road traffic, and heating type. Analysis of the ratios of 1-hydroxynaphthalene to 2-hydroxynaphthalene for subgroups with high cotinine level (3-fold increase) or moderate to high chocolate consumption (1.7–3.4-fold increase) was suggestive of exposure to carbaryl, a pesticide not permitted for use in the EU since 2007. Differences in the burden of PAH metabolites between East and West Germany was investigated using subgroup analysis. Consuming smoked food was both more prevalent (p = 0.009) and likely to contribute to a greater 1-hydroxynaphthalene burden in the East German subsample. Gaining a better understanding of exposure factors, especially using population data from a large number of locations, will guide policy makers and regulators in strengthening protective measures. © 2026 The Authors.
Zielstellung: Ziel des Vorhabens ist es, die in GerES VI gewonnenen HBM-Proben der Erwachsenen auf Schadstoffe zu analysieren, die besondere Gesundheitsrelevanz aufweisen und denen die erwachsene Bevölkerung im nennenswerten Maße ausgesetzt ist (wie beispielsweise Metalle, Weichmacher, polyaromatische Kohlenwasserstoffe, Pyrrolidone, umweltrelevante Phenole). Darüber hinaus sollen im Vorhaben Substanzen, für die im Rahmen der BMU/VCI-Kooperation zur Förderung des Human-Biomonitorings analytische Methoden entwickelt wurden, in den HBM-Proben sowohl des aktuellen GerES VI, als auch in den archivierten HBM-Proben von GerES V analysiert werden. Output: Zusammen mit den Ergebnissen der Befragung der teilnehmenden Personen liefern die Analysen der Blut- und Urinproben belastbare, repräsentative Informationen zur Belastung der in Deutschland lebenden Bevölkerung mit zahlreichen gesundheitsrelevanten Substanzen.
Passive smoking is a preventable and significant cause of many serious health problems, with children being particularly at risk. In the fifth German Environmental Survey (GerES V), conducted from 2014 to 2017, information reflecting the extent of passive smoke exposure in children and adolescents was collected by interview-based questionnaires and human biomonitoring (HBM) analyses of cotinine in urine from 2260 participants, aged 3-17 years. Based on these population-representative data, we describe current passive smoke exposure stratified by different subgroups and identify specific exposure determinants using multivariate logistic regression. The questionnaire data revealed that 42% of children and adolescents lived with at least one smoker in the household. Quantifiable concentrations of cotinine could be detected in 56% of the participants. The overall median concentration of cotinine was 0.2(micro)g/L, with children and adolescents of low socioeconomic status found to be a group particularly affected by passive smoke with higher cotinine concentrations (median=1.2(micro)g/L). In the multiple analysis, the most significant predictor of cotinine levels derived from the questionnaire was passive smoking at home (odds ratio (OR) 13.07 [95CI: 4.65, 36.70]). However, parental smoking and passive smoking among friends and relatives could also be identified as independent factors influencing elevated cotinine levels. The comparison between the previous cycle GerES IV (2003-2006) on 3-14-year-olds and GerES V shows that tobacco smoke exposure of children decreased significantly. This decrease is likely an effect of extensive non-smoker protection laws being enforced 2007-2008 on federal and state level. This is reflected by a halving of urinary cotinine concentrations. Nevertheless, our results indicate that passive smoke is still a relevant source of harmful pollutants for many children and adolescents in Germany, and thus support the need for further efforts to reduce passive smoke exposure, especially in the private environment. © 2022 The Authors.
Starting in 2002, regulations and legislative amendments in Germany focused on the non-smoker protection with several measures to reduce exposure to secondhand tobacco smoke (SHS). The present work aimed to evaluate the relationship between polycyclic aromatic hydrocarbons (PAHs) and SHS exposure and to determine to which extent enforced non-smoking regulations and smoking bans affected the exposure of the non-smoking population in Germany since their implementation in the early 2000s until today. For this purpose, cotinine and selected monohydroxylated PAHs (OH-PAHs) were analyzed by means of (UP)LC-MS/MS in 510 24-h-urine samples of the Environmental Specimen Bank collected over a time span of 24 years from 1995 to 2019. Median urinary cotinine levels were found to steadily and significantly decline by 82% from 1995 to 2019. A significant decrease of urinary 3-hydroxybenzo[a]pyrene (19%), 1-OH-pyrene (39%), 1-naphthol (66%), 1-(17%), 2-(25%), and 3-OH-phenanthrene (22%) was also observed throughout the same time span. The decline in urinary levels of cotinine and several OH-PAHs can most likely be attributed to smoking bans and regulations limiting SHS and PAH exposure. This study therefore emphasizes the relevance of human biomonitoring to investigate the exposure of humans to chemicals of concern, assess the effectiveness of regulatory measures, and help policies to enforce provisions to protect public health. © 2022 The Authors
Im Rahmen des 7. Forschungsrahmenprogramm der EU wurde die konkrete Planung einer Studie für ein EU-weites Human-Biomonitoring (HBM) beantragt (COPHES). Das UBA ist wieder Work-Package-Leader. In 17 EU-Ländern wird jeweils ein HBM-Pilotstudie (DEMOCOPHES) durchgeführt. Die chemische Analytik dieser Pilotstudie umfasst neben der Analyse der Gehalte an Cotinin und Phthalatmetaboliten im Urin und der Bestimmung von Quecksilber in Kopfhaaren auch die Bestimmung von Cadmium und Creatinin im Urin. (120 Mutter-Kind-Paare). Eine parallele Bestimmung von Cadmium im Urin von Frauen und deren Kindern ist bisher noch nicht erfolgt. Die chemische Analytik soll in Absprache mit dem UBA entsprechend der in COPHES zu entwickelnden Vorgaben und Guidelines erfolgen. Wesentliches Ergebnis wird ein Erfahrungsbericht sein, der vom UBA in die internationale Arbeit eingespeist wird.
Die europäische Initiative HBM4EU hat zum Ziel, die Datenlage zum Human-Biomonitoring in der EU anzugleichen und die gesundheitlichen Folgen der Schadstoffbelastung besser zu verstehen - durch Zusammenführung bereits vorhandener Daten und Durchführung gemeinsamer Studien. So sollen Informationen zum sicheren Chemikalienmanagement gewonnen werden, um die Gesundheit der Europäer zu schützen. Die in der Initiative im Jahr 2018 als prioritär benannten Stoffe sollen im Rahmen des Vorhabens in GerES-Proben und den Umweltprobenbank (UPB)-Proben gemessen werden. Ziel dieses Vorhabens ist die Analyse von GerES-Proben auf Flammschutzmittel und von UPB-Proben auf polyzyklische aromatische Kohlenwasserstoffe (PAKs) und Cotinin. Dadurch sollen Datenlücken für den europäischen Bereich geschlossen und länderübergreifende Studien und Auswertungen ermöglicht werden.

In 17 europäischen Ländern startete im August 2011 eine Studie zur Schadstoffbelastung der Menschen – DEMOCOPHES –, an der etwa 4000 Mütter und Kindern aus verschiedenen Ländern der Europäischen Union (EU) teilnehmen werden. Diese einheitliche Untersuchung der Schadstoffbelastung der Menschen hatte die EU in der Vergangenheit in ihrem Aktionsplan Umwelt und Gesundheit gefordert. Den deutschen Studienteil leitet das Umweltbundesamt (UBA). In seinem Auftrag werden bei zufällig ausgewählten Mutter-Kind-Paaren aus Bochum und dem Hochsauerlandkreis (Nordrhein-Westfalen) die Schadstoffe Quecksilber, Cadmium, Cotinin und Phthalate (Weichmacher) gemessen.
The aim of this study was to explore data from the DEMOCOPHES study population for Romania, Portugal and Poland, in order to assess smoking patterns and the extent of ETS exposure and compare the national study samples with reference to the respective anti-smoking laws. The subset of the DEMOCOPHES study sample consisted of 360 children and their mothers (120 in each of the three countries - Romania (RO), Portugal (PT) and Poland (PL). Smoking was assessed using a detailed questionnaire for the participants, which addresses both active and passive smoking.<BR>This assessment uses exposure-relevant questionnaire data, in particular on the home environment and residence, socio-demographic characteristics, lifestyle such as nutrition, smoking behavior, other exposure-relevant behavior and occupational history, as well as urinary cotinine and creatinine measurements. We performed general statistical analysis and innovative receiver operating characteristic (ROC) curve analyses.<BR>Smoking prevalence as evaluated by the questionnaire was generally high, and higher than official statistics, which suggests some under-reporting in the countries, particularly in Romania. Urinary cotinine levels provided biochemical confirmation of the high and similar smoking prevalence for the three countries. Concerning ETS exposure, Romania presented significantly higher levels, for children as well as for non-smoking mothers, with Portugal showing significantly lower levels. Compared to non-smoking mothers, the children showed relatively higher ETS exposure levels in all three countries.<BR>The established country-specific optimal cut-off values in urinary cotinine to distinguish smokers from non-smokers vary more than those to discriminate ETS exposure extent in non-smoking mothers and children. Although different between countries, these values are a valuable output to monitor effectiveness of both national antismoking laws and educational programs in the three countries.<BR>The findings of this study point to the urgent need for stronger, more effective and well enforced smoke-free legislation in the three countries.<BR>Quelle: http://www.sciencedirect.com/
In der Zeit vom Februar bis zum Juni 1986 wurden 100 nichtrauchende Frauen im Alter zwischen 40 und 65 Jahren in Bremen detailliert zu ihrer Belastung durch eine Exposition gegenueber Passivrauchen waehrend der letzten 8 Tage befragt. Als Erhebungsinstrument wurde die deutsche Uebersetzung eines von der IARC (international Agency for Research on Cancer) im Rahmen einer internationalen Studie eingesetzten Fragebogens verwendet. Darueber hinaus wurde von jeder interviewten Frau eine Urinprobe im Labor der American Health Foundation in New York analysiert. Als wesentlicher Marker fuer eine Passivrauch-Exposition wurde der hauptsaechliche Metabolit des Nikotins, das Kotinin, bestimmt. Darueber hinaus wurde eine Standardisierung auf das Kreatinin vorgenommen. - Durch die Analyse der Fragebogenangaben sowie die zusaetzliche Beruecksichtigung der biologischen Variablen konnten trotz des relativ kleinen Untersuchungskollektives erste Hinweise auf den Anteil der einzelnen Belastungsquellen an der Gesamtexposition gewonnen werden. Darueber hinaus wurde die Frage nach der Verlaesslichkeit retrospektiv erhobener Expositionsangaben zum Passivrauchen untersucht. Hierbei zeigte sich, dass der private Bereich einen Anteil von mehr als 60 Prozent an der Gesamtexposition des Untersuchungskollektives ausmacht. Als wesentliche Belastungsquelle muss hierbei der rauchende Partner angesehen werden. Bei berufstaetigen Frauen spielt darueber hinaus die Belastung am Arbeitsplatz eine wesentliche Rolle, zumindest soweit sie ueber den Fragebogen erfasst wird...
